NOTE TO GROUPS:

1. Group Leaders may purchase all tickets for group together.
2. Participation forms must be filled out for EVERY participant.  Simply fill these out by clicking "Assign Ticket" button on each ticket, and entering the name of the PARTICIPANT and email for the participant (or participant parent/guardian.) They will fill out all necessary forms. When complete, tickets will have the participant's name on them.  NOTE: Tickets may be assigned at a later time after purchase.  
3. Submit your Acknowledgement & Assurance Form to [email protected]. 
   • NOTE: This form requires your written attestation that all adults have completed their background checks.  Participation is restricted to groups with this form completed.  Fillable pdf form can be found in the DOWNLOADS section or in the link below. Please fill digitally, save, and return via email to [email protected]. All adults attending must have background checks performed prior to the event.

ALL steps must be submitted to KentuckyWMU before the participant can participate in programming.
